Probably disc, generally downloaded ISO's are pretty good (not bullet proof but generally A.O.k).

if you can get a setup disc for your chip try reducing the speed a little. I've never had to do this as my burns are pretty much bombproof in terms of reliability, but it could help.

ridisc aren't nearly as good as some places/forums would have you believe. Try some more expensive media , buy singles from places like Maplin to experiment with or a sample pack from SVP before comitting to a bulkier purchase.
I haven't used anything but Verbatim or Taiyo Yudens for a couple of years now as the likes of Ritek fell off the scale in terms of quality control.

Try checking your media for PI/PO errors with DVDInfoPro or Nero see what sort of scores you get, if errors rates are very high 100% change your discs and or burner. I see you're using an NEC 3450 these are reasonable burners so check firmware for latest revision.

Just to add my expeience: I use Ritek Go5's burnt at 4x / 6x and haven't had a single fail yet using IMGburn - burnt around 70 games so far. D/L iso's are usually fine, make sure they're the correct region if your chip isn't capable of playing both without patching.

Also not all Ridisc are Ritek so check which dye you have.
