What would the average temperature for an AMD XP2800 Barton .
Mines running at 53 oC when not doing a lot? Don't have any casing fans.
That seems just a little too high for a idle temp. Try getting some case fans.

I would normally have 2 case fans for this CPU, one from front that take in air, then one at the back of case taking out the warm air.

Avoiding the temptation to bring the obvious joke in here (As has been done soooo many times), you actually want both.
The ideal setup is to bring air in at the front and/or side and then exhaust it out the back/top. You also want to balance the airflow as much as possible as well.

I would fill in every hole in the case with fans, if you are worried about the noise you can wire them all at 7v to keep it quiet.
Front: Suck
Back: Blow
Side: Suck
Top: Blow

As there is only one place on the case to fit a fan I presume I have to drill the front or a side panel to fit another?? Back to the original question. What should the idle temperature be?

You don't have to, but it would help. (There are other ways to bring the temp down as well)
Your idle temperature should be around... 35-40 degrees I would say. It really is dependant on many variables however. Things like ambient temperature of the room etc can have a massive impact on the actual temperature of the CPU.

maybe get a new case? I've never seen a case with only one fan hole before. Or you can cut holes but cases are cheap these days it might be easier to just upgrade.

Case isn't even a year old. But if one fan doesn't do it will have to consider a new box. Thanks to everyone.
